Pain Diary

Do you remember how you felt last week?

How about yesterday? Understanding when you were in pain, and what you were doing is important. Keeping a pain diary will help you track when you take your medicine, your pain levels, and daily activities. This information will be helpful in your discussions with your doctor.

How to use the diary

Follow the instructions included in the pain diary. The diary is intended to be used each day for the next seven days. Use a new page for each day and bring the diary with you to your next appointment with your doctor. When you feel pain, write down the relevant information in the diary, including time of day you felt the pain, the location of the pain in your body, a description of how the pain feels, what medications you took, and your activity level at the time you felt the pain. Rate your pain on a scale of 0-10, where 0 is no pain and 10 is the worst possible pain.

If you wish to keep a diary for longer than seven days, simply print out as many copies as you need.
